Reminders, Recalls & Follow-up appointments
We ask patients to book follow-up appointments to review any test results at the conclusion of your GP consult. You may also be asked to ring us back to make sure your results have arrived and ascertain whether there is any need to be reviewed before your scheduled next appointment. Please note that results can only be given to you by your GP. Please advise your doctor if you are unable to proceed with any recommendations (referral, management plans, etc) so that other arrangements can be made for you. Our Facility uses a reminder system like HotDoc to improve the quality of your health care. The system sends reminders by mail, telephone or SMS for procedures such as vaccinations, cancer screening such as CST & bowel, and other health reviews that you may be due for.
Transfer of Care
You may choose a new doctor, for whom you will need us to transfer your clinical notes to. We can only do this with your signed consent authorising us to release your medical record to your new treating physician, fees may also apply for which you are responsible to pay. In the event of a minor who is aged 14 years and under, a guardian/parent must also co-sign the authority . In many cases we will contact you directly to confirm your consent, particularly if the signature is not legible or if it remains unclear who has signed the consent form. We endeavour to finalise all transfer requests within 2 weeks but occasionally it may take up to 30 days.
ID checks
We may ask you to confirm your ID, even if we see you regularly. This usually requires you to pass 3 ID checks before we can assist you. In the event you update any of your ID documents, be sure to present the updated copy to our reception at your next appointment to ensure that all future ID checks are completed properly.
ZERO tolerance policy
We are a zero tolerance facility, no exceptions.
